To make summer camp easier to register for, you can download all the forms you'll need right from our website. To register for summer camp, you must have an enrollment application on file with us. You can submit it along with the registration form and a completed copy of each of the forms below. A $100 deposit is due with the registration form for each week you request. These forms will be posted by January 15, 2011. If you would like them to be mailed to you instead, please call 262.275.5753.
